Fidelity HSA Rollover Form

Anyone know where to locate? I checked the form page and do not see it. My plan is to rollover my HSA from Saturna to avoid the $75 transfer out fee.

thanks, but I don't want to do a transfer of assets.
I called in, it's actually just their basic deposit form, just check 60 day rollover. Nice chat with a HSA rep, he stated they are taking a lot of business from Lively and others.

Can you provide link to form you used? I was all over website trying to find forms and got lost...

That would be to transfer assets, but there is a $75 transfer out fee from Saturna. You can request a check and then do a 60 day rollover...no fees.

I asked this very question a couple of weeks ago in the message center, and they wrote back with a few options for completing a rollover:

1. Mail a check in with the deposit slip indicating rollover.
2. Drop the check and deposit slip off at my Fidelity branch in Cincinnati. (At least one person in another thread said that his/her branch wouldn't accept a rollover check.)
3. Call the HSA team and have them pull it in from a linked checking account and code it as a rollover.

This is the form I used too. Just check the 60 day rollover box on the form.

Thanks all. Employer with PayFlex so will be using this to side step $25 transfer out fee
