Is there a Global Small Cap ETF/fund?

No. At least not an indexed one. The Russel etf's come out in June and I think they will be the first.

A mix of small and midcap is contained in
Vanguard FTSE All-World ex-US Small-Cap Index Fund Investor Shares (VFSVX) or if you have institutional size money
Vanguard FTSE All-World ex-US Small-Cap Index Fund Institutional Shares (VFSNX).

Over here YDNAL recommended this fund to balance someone whose best 401(k) international option is Dodge & Cox's international which is large cap only.

If you are trying to balance against a large cap international, that looks to me like it would work. If you already have large and mid cap international, that might not be what you want.

Only managed funds like TEMGX with a load and high ER and maybe 100 stocks.

The best solution is probably a smallcap US etf like VB and FTSE ex-US Smallcap (VSS) which has EM SC too. That will get you about 4400 stocks.
