Can a group of people (i.e., 3 or more) open a Vanguard account?

We have a VG account with three names on it. No problems.

Sure.

However at this stage most people would move away from a joint account and would open up some type of partnership. Try looking up Investment Clubs. This is a fairly standard method.
Former brokerage operations & mutual fund accountant. I hate risk, which is why I study and embrace it.

Organization Accounts might meet your need.

Consider opening an organization account for a legally established company—including:

Corporations
Partnerships
Limited liability companies
Sole proprietorships
Endowments and foundations
Estates
Professional associations
Unincorporated enterprises

This may sound dumb, but, how do you do it? I only saw an option for a joint account (i.e., 2 people).

We just submitted a paper form to add the third person. There maybe other ways. It’s been about 7 years since we did this.
