Sorry yes I probably should have clarified..

The first problem is that the UI form was filled out 3 days before we helped her fill out the PUA form, which means someone else did this.

The second problem here is instead of "Jane Smith" the application says "Smith Jane"

Thanks -- we did contact them but they didn't respond (two inquiries over the past 3 weeks). I understand they could be backed up.

Is it OK that they filled it out for her? I thought only individuals were to file their own UI claims?

The manual says it needs to be replaced in 80k miles. We have 112k miles on the car. Are you sure about that? I have a slightly newer Mazda and the manual does not call for replacement at all, only inspections every xx miles. I'm almost certain the 2014 manuals are the same...requiring inspections ...
