Partners generate revenue. Might be allocated based on some sort of formula to account for productivity. Corporate expenses are paid from the pot. Money after that is divided according to ownership percentage. Expenses are taken out individually since they can vary dramatically.

Just curious — would it be financially risky for the OP and his wife to both completely retire if they wanted to? They have >33x annual expenses saved. Maybe financially ok, but losing physician identity and profession at such a young age is a big risk. Yep. Crush it for a few more years. Pay people to take care of the mundane tasks in life. If your soul is truly crushed see if you can work part-time. Once your kids are in school you will be bored unless you truly have hobbies that can keep you happy and occupied. You have come a long way to call it quits at such an early age. Go out on top when you can truly cruise. Kids are expensive. So is a condo in Deer Valley. The corporitization of medicine is soul-crushing. Mid-level encroachment (...

be careful of costs and expenses. If I had to do it all over again would use an independent consultant, if such a thing exists. Realize you can't just pay yourself a pittance for a salary, it has to be reasonable - and the money you stash away each year depends on your age and actuarial assumptions. Finally, you can't just invest 100% in the index 500 fund or whatever and hope to let it rip - investments need to be reasonable so that if the market tanks you aren't up a creek without a paddle because you can get locked into contributions and also be required to make up shortfalls. Most folks seem to keep the plan 5-7 or 10 years and then roll the funds out and maybe start again.

If the anesthesiologist is an independent contractor or working at the surgicenter or hospital as part of a contracted group that has contracted with the hospital or surgicenter to provide anesthesia services, then anesthesia contracts with insurers will be separate from the contract with the hospital or surgicenter. The hospital or surgicenter may or may not require their anesthesiology service to accept in-network fees depending on the local market for anesthesia services. And this is where the problem is. All I see in my area is anesthesia groups signing contracts with hospitals. After the contract is signed there is no need to be in network (unless it's in the contract to do so). So, hospitals share in having out of network anesthesiol...

That being said, I agree that your interpretation of the wRVU scam is incorrect. You are not going to get immediate work credit just because you submitted a CPT code. There are a lot of steps between submitting that code and it actually being sent through to insurance and then awhile yet before the charges are actually reimbursed. A one to two month lag time is common. The advantageous way to leave would to leave after the bonus is paid out for the prior fiscal year. It's not a scam.

The hospital might be "billing you out" at $12M but the amount they actually *receive* from third party insurers is typically a small fraction of what they bill. Typical bills have unrealistically high and inflated "sticker prices" that essentially nobody pays. Then you have the uninsured patients with little or no assets. It does not matter what you bill them because you can't get blood from a stone.
I am interested to know how the OP got to the 12M number. This would require seeing the "books" and/or the raw billing numbers. I don't think this info would be readily available in the situation described.
