You are expendable. Don't get cocky. At that salary level you must perform. Is it worth it?

Yes, I used the Ohio 529 plan for both my kids. Fees were reasonable and investment choices included Vanguard funds. Wish you could deduct more than 2k for your State taxes, tho.

Recently went from a Honda CR-V to 2018 Grand Touring CX-5. Thoughts: Internal appointments much nicer on CX-5. CR-V felt a bit utilitarian.Navigation system works very well, but takes getting used to. You update the navigation SD card every 6 months. Free for first 3 years. Styling on Mazda much sportier. Drives very nicely. Engine is a little rough, but minimal internal noise. Mileage so-so, but CR-V was nothing to brag about either, Once you use the heads-up display, you can never go back. Multitude of warning lights and remainders. Kinda feels like driving in a video game. The dealer essentially wouldn't deal on the CR-V price with no special financing. Mazda gave us 0% financing for 60 months. Wife likes the Mazda more than she did the...

Agree, no good answer, at least for the self-employed or with pre-existing conditions. My 7k deductible plan costs me 30k per year and is going up 30% per year. The insurance companies are pricing themselves out of the Market for private insurance. At least my HSA and premiums are deductible above the line.
I believe there will be significant announcements regarding healthcare over the next 6 weeks. Maybe expanding HSA contributions, AHP's, HSA-compliant Health care ministries. I am holding my breath. A race to Medicare.

In the face of a medical catastrophe, sign up for Obamacare, realizing you can only do this once a year. Self-insure to this point or try a health care ministry. Consider a Healthcare Reimbursement arrangement.

I might call Chokoloskee rustic, but not scenic. Everglade City is the only town actually inside the Everglades. Not very large, mostly airboat rides and charters. Camellia Street Grill is kinda interesting in Everglade City. Stan's can really rock in Goodland on certain days. Stop at the visitor center just into Big Cypress and try to spot a manatee in the pond around back. Oasis Rest is gator central.

Kirby Storter boardwalk off Route 41 in November (high water) can be fabulous and one of my favorite places. Big Cypress Park is not quite to the everglades but very interesting. Canoe/kayak Turner river. Oasis Rest for the largest gators you will ever see. We stopped at Shark Valley last March and saw all sorts of wildlife. The Mangrove kayak off Marco Island is fun. Air boat ride out of Everglade City is fun. Camille's for lunch. From Naples, the Everglades National Park is a 3 hour drive. You have to go to the opposite side of the State and then turn south.
No real need to take I-75 unless you want to get to Miami/Ft. Lauderdale fast.

Stop thinking you need a "degree" to enter a certain field. The extra training will be expensive, unnecessary and probably obsolete. I have a physician friend who makes good bucks doing EHR consulting and he practices full time. He doesn't have any computer science background, just interest in the subject. So many unfilled niches in Medicine. So many opportunities. Keep your eyes open. Attend conferences like SEAK. They are probably the most "outside-the-box" thinkers for physicians.

About your age I started looking for opportunities to work one day a week or from home. Actively went out to seek new positions and challenges. Consider a short sabbatical to get your head screwed on straight. Be prompt and cheerful in your new jobs. Never a prima donna. Become a small business. Don't think of it as "retirement" which is an outmoded concept. You are a "financially independent"physician who chooses to work where and when you want.

Is retirement defined as a lack of earned income or the end of the main vocation in your life? I have too many uses for earned income to ever retire, if I can help it. I view my retirement savings as a backstop, giving me the ability to move on to another vocation or interest as I see fit. The camaraderie of the workplace is a precious thing. Working at a profession at the peak of your abilities is about the most fun you can have with your pants on.

I follow the Larry portfolio. You are in a risky portion of your life...a large Market drop could endanger your retirement. To me, the US Total Market fund is essentially the S&P500.

If you are only tax gain harvesting right up to the top of the 15% bracket, you have no use for tax losses. You are already in the lowest cap gains bracket (0% rate) you can get. Do the same next year and so on until all the gains are used up. The funds/ETFs you sell for gains harvesting could just as well be up next year. I am using TGH to re-balance out of large cap stock funds into SCV and Intl. If I didn't need to re-balance, I would just re-invest back into the same fund and reset my basis. Book the losses when they occur which is not often lately.

I am currently 30/70 stocks/high quality bonds with half of total assets in a taxable account. Stocks are 50% SCV, 30% intl./30% S&P500.

I am lowering my RMDs either way...Roth conversion vs. taxable account.
If the Market goes up predictably over the next decade, then yes, I will have more after-tax money in the Roth. If it doesn't, all I can do is re-characterize. In a taxable account, I seem to have more options. I can gift low basis stock and TLH all day long.

Could you receive some income as a W2 employee, say up to the SS limits? Then form an LLC and be compensated as say, an EHR consultant and contribute these funds into a solo401k.

I am in exactly your situation. I have the funds to pay off my 3.25% HELOC at any time, but don't. My Home is not at risk. After taxes and inflation, these funds appear to be near free money. The asset protection is also nice. I am sure the bank would love it if I paid it off. If long term mortgage rates go to 2% (and fees are are reasonable) maybe I'll refinance.

I feel your pain.
Are there other concierge physicians in the area? Generally concierge physicians are personable and accommodating; almost as useful as being "competent." They must value high quality care if they have gone concierge. Consider calling independent Primary care docs in your area. Would you be willing to pay to interview the doc and see if it is a fit? A physician with 30 years of experience, is likely to be competent, but I recognize lay people may have difficulty determining competence (remembering it is not an all-or-nothing thing.)

A physician with 30 years of experience, is likely to be competent, but I recognize lay people may have difficulty determining competence (remembering it is not an all-or-nothing thing.)

I am a former Primary care physician. I was always surprised how patients would throw away a 20 year relationship for a $5 co-pay. My Practice had 5 full time employees just to deal with insurance issues. A very poor business model.
Personally, I believe a generalist physician that you know and trust is a highly valuable asset. It is an old fashioned idea that most people don't subscribe to, currently.
When a physician physician goes concierge, he/she is usually already established in the community and popular. I have seen estimates that new concierge physicians will lose 60-90 % of their patients in the transition.
Could you stay on as cash only?

My portfolio draw down is more influenced by taxes than anything else. I use the Larry portfolio with a rising equity glidepath. Much of the Prime Harvesting method appears to be momentum play i.e. "don't sell your stocks too early."

The desire for dividend income confuses me. Doesn't the share price/NAV drop after a dividend? Now you have shares worth less and a taxable event. If one invests in high dividend stocks, doesn't that result in low diversification? Doesn't the Market know about the dividend and raise the original share price?
I can use capital losses to offset capital gains, but there is no such thing as a "dividend loss."
