403b vs 457 in general I'm not sure, I'm not an expert.

In Ohio, 403b are sold by salespeople, usually have high fees, confusing terms, etc. Ohio457 has Vanguard funds at a lower ER than I can get on my own at Vanguard. It's a win-win.

What state are you teaching in? Is a 457 an option?

In Ohio, the Ohio457 has super low fees and is almost like a secret since they don't advertise, have salespeople, etc.

I opened an 11 Month no penalty at 1.75%, transferred from our Ally Savings account, online in about 2 minutes. Meanwhile, the wait to talk to someone in the upper right moved from 31 minutes to 27.

I would call the insurance company first, explain the situation, push a little and listen to their advice. Based on that you could appeal to insurance, negotiate with the doctor, etc.
