Recently returned from a Europe trip and Schwab and Fidelity debit cards both worked well (with no ATM fees, nor additional foreign exchange transaction fees)

PAMF has been playing the game quite effectively. They became a monopoly by buying Sutter Health, and so for most areas in the Bay Area, they're as you put it the monopoly.
