Employers do have the option of withholding at a flat 22% or combining the bonus with any other wage payments made that week (like regular salary check) and withholding based on W-4. I don't see this in the instructions, but the tax software I used put the figures from my 1040 line 2a, 3a, 4a and 5a on the CA adjustments schedule by the radial dials. And the amounts from form 1040 lines 2b, 3b, 4b and 5b were on CA Adjustments column A.

83b elections can be made for restricted stock grants, but not for restricted stock units. Are you sure you received RSU? If so when were they granted, when did they vest, and when did you exercise them?

I think it comes down to how comfortable you are that if you/spouse lost their job, had a medical emergency, etc. would you be able to keep paying the higher 15 year payment amount? Unless you are comfortable you can stay afloat, going with the 30 year option and paying as a 15 year loan makes sense...

In addition to the comments above, I think you should always run a loan amortization schedule and see what your total remaining payments are under the old loan and compare to total payments under the new loan. Remember that in the new loan you're starting your amortization over and have 360 more pay...
