I had a good experience with Enchanted Diamonds. The online shop is way easier to use than Blue Nile, and has a lot more data. Plus the advantage of being in NYC is that their office is in midtown, and you can go pick it up, get free adjustments/cleanings for life, etc. Their service is excellent.

Brooks Bros Milano fit if you're a marathoner-type "athletic," or Regent fit if you're more of a lifter-type "athletic." I tend to go with the latter (I generally distrust most brands' generic "slim fit" cuts because they're usually far too narrow across the shoulders.)
