Are you eligible for STRS? They may have lower cost. Also when you reach $5000 you could switch to self-directed brokerage account with Charles Schwab which may have lower expenses. The $60 per year administration fee is not unreasonable - I know that it sounds outrageous when you only have a few hundred bucks but when you have a few $100k it won't be a big deal. The match alone is worth it - as long as you plan on staying at that position.

If this doctor's office cannot follow Medicare procedures, I see no reason why I should have to go through all of this nonsense. I'm paying three times more for Medicare than when I was employed and now I have to go through this bureaucratic nightmare and it amazes me that some people seem to like Medicare!!! I'm sorry you're having this frustration, but you are not the only one. This is the exact reason why Medicare is being slowly marginalized in private practice settings and will soon be left to institutional clinics to manage these patients. Well, I have received a letter from the Appeal Board that part of the claim is denied and there is still a dispute about some of it. Basically, it seems that my doctor followed the Wellness Test that my employer mandated for many years. But he did not check all of the boxes that were required by Medicare as he knows me and my medical history so well. The Doctor's office told me that I would not have to pay and it was covered by Medicare. For now, I have had enough of this dispute. I haven't been billed for anything yet and I'm not going to call Medicare and get involved. If the doctor's office does bill me I'll pay but will probably consider changing doctors. Well, I have received a letter from the Appeal Board that part of the claim is denied and there is still a dispute about some of it. Basically, it seems that my doctor followed the Wellness Test that my employer mandated for many years. In the proxy information, Vanguard's fund board of trustees offers their recommendation on how to vote for each proposal. The first 6 proposals were set forth by Vanguard. They recommend voting for the first 6 proposals. Proposal 7 was made by shareholders. The funds board of trustees recommends voting against proposal 7.

I find it interesting that 60+% of participants were automatically enrolled in their plans and the majority of those were professionally managed. I think that it is a good trend that more and more employers are having new hires have to 'opt out' of the retirement plans. Some plans (such as STRS in Ohio) have nonnegotiable contribution rates too (14%). This may skew the 10% max contribution number as salaries >126k at 14% would max out the 401k/403b

At your income I would avoid taxable until you max out the IRA account especially since you don't have a work 401k. I would put the $5500 in the 2017 roth IRA. If you AA is 90/10 then you could use the vanguard target retirement 2050 fund. The ER is 0.16 compared to betterment's 0.25.

Thank you. I think there are two issues: 1. I was coded for (G0439) instead of welcome to medicare exam (G0402). These problems arise as I retired at 73 instead of 65. 2. The Welcome to Medicare is supposed to be for screenings and possibly shots. There did not appear to be any tests done on my Welcome to Medicare. I have been with this doctor for many years and gives me screenings such as EKG occasionally. He felt it was not necessary to do it this time. While I was employed there was never any problem with billing and we were virtually forced to go on these annual wellness tests or else we would be on a higher rate. If we did not go we constantly got reminders from the Health care provider that the deadline was approaching. I suppose tha...

I would recommending going with an entry level dslr and a prime lens. For example - you can get a canon rebel t6 and a 50mm prime 1.8 lens for ~$600. Else you could get an iPhone 7 plus and get the same effect as they have two cameras

If you're drinking red wine for better health I would recommend water instead. The main researcher for resveratrol was Dipak Das at UConn. He published multiple papers about the health benefits of resveratrol. Before he died, he was charged with 145 counts of fabrication and falsification of data. More than 20 of his papers have been retracted. He was fired from UConn. I would recommend cardiovascular exercise if you are looking to raise your HDL

It looks like they billed it as an annual medicare wellness (G0439) instead of welcome to medicare exam (G0402). The welcome to medicare exam includes EKG, vision check, and hearing screen in addition to the normal annual medicare wellness questions. If they drew blood work then they needed to add a modifier for it to be covered. The markup for CT and MRI at a hospital are frequently 4x what the cost is at an outpatient private imaging suite. For example - MRI at hospital in my area frequently runs $2-4k. MRI at outpatient privately owned imaging facility $650. Same with CT - usually at least 4x cheaper at an outside privately owned imaging facility. This also applies with laboratory work but less so - you can ask your insurance if getting blood work done through a labcorp or quest drawing facility would be cheaper then to go through the hospital lab

The standard in my location is a 15% discount for cash paying patients. This is off everything - the visit fee, blood work, procedure fee, etc. It would be expected that the patient pay in full before they leave. You can get a quote if specific testing or lab work is needed. I'm not sure if the 15% off is pretty typical nationwide or not. I agree that physicians overall have very little say in the billing process other than potentially downcoding or just not billing for certain services

Yikes thats a lot of med school debt. I'd hold off on the house for a few more years and pay down the loans. Is your wife working 0.5 FTE? Or is she in academic medicine? I don't practice in NC but her salary seems extremely low. She may want to look around for a new position as she should be able to earn twice what she is earning now

How old are you? If you have commercial insurance, an annual physical is supposed to be covered 100% per the ACA. Included blood work differs by the insurance company but should include glucose and cholesterol screening. Almost all plans still cover annual PSA testing too (if you are > 40). If you had other issues that were addressed the doctor may have charged another visit on top of the preventive E&M or placed a modifier on the bill. I don't think the cost is worth your hassle but you could ask what E&M code was used and if any modifiers were added to find out more about why it wasn't covered 100%

I wouldn't say there is a high probability, but the probability is at least non-zero! Regarding the contributions, if I want to contribute the maximum per year, I would effectively be paying 9K pre-tax each year and the company would match that by depositing 9K into my 401K. Now let's say I leave after year 1. Would the company withdraw 9K from my 401K? That's what I meant by being left with an effective 50% contribution which would be quite detrimental for my retirement planning! The maximum pretax contribution is 18k unless you are in the 'catch-up' phase (the company match is not included in the maximum). If the company matches 1:1 or 100% then you will have 36k in your account after making the maximum contribution. That is a good incen...

This is from my experience so YMMV... There is a difference whether it's a plantar wart vs flat wart. The CPT code should've been either 17000 or 17110. Average reimbursement is about $80. The CPT code (and thus reimbursement) is the same no matter how the wart is destroyed (acid, freezing, burning). The visit code should likely be a 99213 unless other items were addressed. Sometimes a modifier will be added. Total cost should be around $200. I'd try to find out which codes were used. This was likely billed incorrectly with a lesion excision code. Good luck

For what it's worth, I'm 33 and would like to retire at 53 too. I'm at 80/20. Everyone has their own risk tolerance. I plan on increasing bonds by 1% per year to reach 60/40 for retirement. It seems like every financial institution has a risk calculator questionnaire. You may want to try several of them if you still have second thoughts on your AA

On goodwill's website they list a tax deduction of $1-3 per hardcover and $0.75-2 for paperback. The tax deduction is probably better then I could get if I sold my old books in a yard sale or online (and is more convenient)

Did your income level change? If not, make sure to pay 100% of the taxes you paid last year (or 110% if you make more than 150k - look up the "safe-harbor" requirement). Go through form 1040 ES to see what estimated payments you owe. Q4 estimated payments are due 1/17/17. You will be penalized for the first 3 quarters but can avoid Q4 penalty if you pay the federal and state estimated tax by then. Form 2210 is used to calculate the penalties

I personally think Glenfiddich 15 is the best tasting scotch for the money. If your friend's aren't connoisseur's then you may want to just stick with Black, Green, or Gold label and get something else in addition to the scotch. Blue is definitely smoother but not worth the extra $150, IMO

Looks great - you have a lot of good options with the 401k accounts. I'd consider a lifecycle fund for her 401k. You may be better off with changing your 401k to all pretax as you will likely be in a lower tax bracket in retirement, however there are some advantages to having a Roth 401k. If you like the advantages of the Roth 401k there is no problem with keeping 1/3 of your 401k as post tax contributions. I'd calculate to see how close you are to the 28% tax bracket (if putting a little extra in pretax money puts you into the 28% bracket then I'd do all pretax for the 401k). Also make sure you are contributing the 5.5k each to traditional IRAs and then converting to Roth IRA through the backdoor. I'd put the bonds in this as you don't hav...

You have a good 401k. I'd invest in the S&P fund, total international index fund, and total bond fund. I'd then use a Roth IRA through vanguard to add the desired REIT and small cap to reach your desired allocation. It takes more time to reallocate but you'll save more with the lower expense rates

It appears your allocation is skewed toward domestic stock. You need a little more bonds and international to meet your requested allocation. I would simplify the taxable accounts with just total stock, S&P 500, or international index funds. Get rid of the bond funds and individual stocks. You'll be more tax efficient with the bonds in the retirement accounts. You may want to try to see if her 401k custodian would take her rollover IRA. You could then consider a backdoor Roth IRA

So I tried changing from a Roth 401k to a traditional 401k. My HR representative told me I need to do this through T Rowe Price. T Rowe Price said I couldn't change as log as I was still contributing to the 401k (still employed by the company). Has anyone else had trouble changing from Roth 401k to traditional 401k?

Excellent, it looks like I'll be able to roll the rollover Ira back to the company 401k. Thanks for the great advice. One more question - I contributed to the 2015 TIRA after December 31, 2015. Vanguard let me do the backdoor conversion to the Roth (in 2016). Since I missed the December 31st deadline for conversion should I avoid contributing to a TIRA in 2016? Can I convert both 2015 TIRA and 2016 TIRA contributions to a Roth in the same year?

Hi - first time poster here (still have a lot to learn!), Question: Would it be best to take the 10% penalty to cash out a small rollover IRA account to avoid roth IRA conversion taxes? Situation: My wife (age 29) rolled an old 401k worth $4300 into a vanguard rollover IRA account. We don't qualify for roth IRA (33% tax bracket) so I am in the process of converting TIRA to roth IRA. From my understanding the rollover IRA counts as a TIRA and would thus be taxable (however it can't be converted to a roth IRA). I'm considering cashing out the rollover IRA and taking the 10% penalty to hopefully avoid pro rata taxes every year for the next 30 years.
