vanguard bank account issues?

Anyone else have issues with their vanguard funds joint bank account information?

I am unable to see my registered banks for my joint account. It mysteriously disappears when trying to make a new purchase and I am unable to re-add a new account. Mysterious glitch or did I get hacked?
