Shouldn't be difficult, usually the bene form has a place to name a trustee under the xyz trust. They often require a copy of the trust and will require the trust id.

Steve B is right on the money. I recently placed 1,000,000 policy on someone that had melanoma 8 years ago that was "in situ" and at standard rates.

Unlimited caps are a mirage, IMO, one of the leading insurors told me they had NEVER had a client hit their 3 million lifetime cap.

You should seek the advice of a qualified estate attorney. However, if Grandpa is not competent, his will or laws of intestacy will govern how assets are distributed, the family can't change that.

In most all cases, the HDHP will be a better choice if you do a best case worse case scenario, the tax deduction is just a bonus. That is the case in my state anyway.

In reality, home care can be more costly than nursing home care, depending on how many hours of care are needed. I know of a clients father that was spending 100k per year for home care.
