effect on annuity income when spouse enters retirement home

If a spouse enters a retirement home does the state split the annuity income between spouses like the other assets?

My state is not expanding Medicaid under the Affordable Care Act. So I have to withdraw at least $11,500 from my IRA to get a subsidy. If you have an MAGI of less than $11,500 you do not get the subsidy if you live in a state which is not expanding Medicaid.

If you retire and have no income accept for withdrawals from retirement accounts, it would be well worth it to move to a state expanding Medicaid

Anyone considering moving to a state that offers Medicaid expansion under the Affordable Health Car Act, to save on healthcare costs and retire before Medicare kicks in at 65?
