I did not take a withdrawal. So, do I need to send any form to the IRS?

Thank you for your reply. I will call tomorrow to find out more information about the annuity. The life insurance is paid in cash ($300k). Assume monthly expenses to be roughly $2,500-$3,000. Also, I should note that the monthly mortgage payment is $1k, with 20 years remaining.
